Canton Soccer Club - Participation Waiver

It is the purpose of this agreement to exempt, waive and relieve releases from liability for personal injury, property damage, and wrongful death, including if caused by negligence, including the negligence, in any, of releases. “Releases” include the Canton Soccer Club, member teams, event hosts, other participants, coaches, officials, sponsors, advertisers, and each of them, their officers, directors, agents and employees. By signing this waiver, the participant (and participant’s parent(s)/guardian(s), if applicable) waive, release and relinquish any and all claims for liability and causes(s) of action, including for personal injury, property damage or wrongful death occurring to participant, arising out of participation in club events, member team activities, the sport of soccer, and/or activities incidental thereto, whenever or however they occur and for such period said activities may continue, and by this agreement and such claims, rights, and causes of action that participant (and participant’s parent(s)/guardian(s), if applicable) may have are herby waived, released and relinquished, and participant (and participant’s parent(s)/guardian(s), if applicable) does(do) so on behalf of my/our and participant’s heirs, executors, administrative and assigns. In the case of injury or medical emergency and in the event participant, or their parent or guardian, cannot respond at the time of the emergency, the Canton Soccer Club has permission to seek, administer, or have administered whatever first aid or emergency medical care deemed necessary for participant’s welfare, and it is understood that participant, and not the Canton Soccer Club, shall be responsible for any and all charges for such health care services regardless of whether participant’s medical insurance would cover such charges. Furthermore, the participant (and participant’s parent(s)/guardian(s), if applicable) recognize that soccer has a certain degree of risk, and knowingly and voluntarily assume the risk of any injuries, regardless of severity, including death, and all risk of damage to or loss of property which may incur, even if arising from the negligence of the Canton Soccer Club, while participating in the sport of soccer. By signing this waiver, the participant (and participant’s parent(s)/guardian(s), if applicable) certify that there are no medical reasons why the participant cannot safely participate in Canton Soccer Club activities and agree to abide by all Canton Soccer Club policies.
